Private insurers and unlimited spending from big pharma lobbyists ensure US politicians keep those extra 0s right where they are, and that we get worse medical outcomes as a bonus on top.
Besides, the capitalists want us dependent on jobs for healthcare. Much easier to manipulate and exploit the workers that way.
The real trick is convincing a lot of people that this is the best way to handle it.
